To register with HCA, complete each of the following forms and return them to the Registrar onsite or at the address listed in the Registration Checklist.

3. Family Contact Form-Medical Release Waiver
4. Registration Agreement
5. Course Registration Form
6. Code of Conduct
7. HCA Indemnity Agreement

Please Note: All of our classes require a minimum number of students in order to be conducted.
